The Substance Abuse and Mental Health Services Administration (SAMHSA) defines harm reduction as “a proactive and evidence-based approach to reduce the negative personal and public health impacts of behavior associated with alcohol and other substance use.”
Why harm reduction?
If we don’t know when, or if, someone is going to change their pattern of substance use, our goal is to keep them and others as safe as possible. The goal is not to endorse substance use, rather to reduce the harm that an individual can inflict upon themselves or other people. Harm reduction programs and SSP’s (Syringe Service Programs) have been proven to dramatically reduce risk for fatal overdose and other public health concerns as much as 50% (here).
What does harm reduction really look like?
Harm reduction can take on several forms. Ranging from the distribution of Naloxone (Narcan), referrals to care and treatment facilities for interested parties, supply syringe service programs (SSP’s) that prevent the spread of HIV and Hep C, education about overdose risks, and it also helps to reduce stigma for individuals who are actively using.

Rural areas face critical shortages in harm reduction services, with many regions lacking syringe exchange programs, naloxone distribution, and other essential interventions despite high overdose rates. Studies highlight that logistical barriers, stigma, and limited resources prevent rural communities from adequately responding to the opioid crisis.
